Appointment Scheduling

When scheduling an appointment you will be asked for your name, date of birth, home phone number, medical insurance information, and a brief description of why you need to see the doctor.

If you have recently had an X-ray or CT/MRI for this condition, we will ask you to bring the films or disc with you along with all medications or a list of medications that you currently are taking.

If the patient is younger than 18, a parent or guardian must come to the appointment.

We ask that you arrive 30 minutes before the appointment time to allow time for the completion of paperwork. If you need help with the paperwork, please have someone with you for your appointment. You can save time by downloading the forms below and filling them out before your appointment.

Preparing for Your Appointment

In order to best serve you, please bring the following with you to your appointment:

List of ALL medications and supplements taken (It may be helpful just to bring all medication bottles with you on your 1st appointment)

  • Completed material from required patient forms (see below)
  • Insurance cards
  • Any studies or images (X-ray, MRI, Reports) related to your visit

Required Patient Forms

New patients should arrive 30 minutes prior to the appointment time so that we can process the information, create the medical file and place the chart in line for the designated appointment time. If you have a computer and printer, you may download the required forms now. Bring all medications in their original bottles (not a pill box) to your appointment. Be sure to include any over-the-counter medications you current take.

Fees & Billing

All copays are due at the time services are rendered. Surgery fees are collected through the hospital system. Please call  318-212-7841 with questions about your physician bill.

Pay Your Bill Online

You can pay your bill online using Online Bill Pay.

Insurance

Our office accepts most insurance plans and will file the insurance on your behalf. Your insurance coverage is an agreement between you and the insurance carrier; however, the final responsibility for payment of the bill belongs to you.

Release of Information

We are unable to release any information to anyone without your written consent on the required Alternate Contact Form. Your information is privacy-protected.
